what is Home based primary care ?
Home Based Primary Care provides the same level of care you receive in a primary care office setting but in your home, residence, assisted living facility, or skilled nursing facility by a licensed provider, who has received formal education, training, and certifications to provide primary care to you.
We assess, diagnose, and treat acute and chronic illnesses as well as prescribe medications, write orders, interpret labs and diagnostic tests, and issue referrals so that you can receive additional specialized care as appropriate.
Are You Eligible?
We provide health care services to Medicare and Medicaid beneficiaries and other qualifying patients who have difficulty leaving home safely or accessing medical care and meet the necessary requirements set by Medicare, Medicaid, and other insurance companies.
If any of the following apply, you may be eligible to receive medical care at home:
- Difficulty walking, and leaving the home safely
- Shortness of breath caused by exertion
- Cognitive or behavioral impairment
- Leaving home requires a recovery period
- Having 2 or more chronic illness
- Other barriers to quality and equity in primary care
