Bliss Primary Medical Care provides additional layers of  support with Chronic Care Management (CCM)  services to our patients with 2 or more chronic illness. Our focus is to ensure this subset of patients receive additional touch point conversations including  medication, disease management and education, assistance with coordinating community services and resources and much more. 

Bliss Primary Medical Care partners with Master's Prepared Social Workers to assist with patient, family, and community resources. Experienced clinicians help patients holistically mend socially fragmented areas of their life improving  overall  health and wellness of our patients. 

Patients with chronic conditions including hypertension, diabetes, congestive heart failure, lung disease, and chronic infections receive a device appropriate for their diagnosis at home. Each patient is remotely monitored by a registered nurse ongoing. Our goals are to ensure patient condition(s) remain stable, improve quality of care and reduce hospitalizations. 

Bliss Primary Medical Care has a collaborative partnership with  "Carolinas Community Mental Health and Psychiatric Services" to manage patients with Mental and Psychiatric Disorders using a synergistic approach to improve patients overall mental and physical quality of life. 

You do not have to leave the comfort of your home for lab services. We have an excellent partnership with "On The Go Mobile Phlebotomy" and "Assure Lab" to ensure routine and stat labs ordered by one of our providers are obtained and processed timely. 

Scans including x-rays, ultrasounds, EKGs, vascular studies and other similar services can be performed conveniently in your home based on the recommendation of one of our providers. 

We would much rather see you in person, but we realize this is not always possible. Telehealth is an excellent alternative service for our sick patients who may need to  quarantine. We also rely on telehealth services for after-hours, holidays, and emergencies.
